3、班级学风好,班级文化成绩综合评价达年级中等以上水平,中途接任班主任,学生成绩提高幅度较大。

4、班级日常管理到位,班风好,常规考核期末总评分年级排序居中等以上水平。班级无重大责任事故,班级受记大过以上(含记大过)处分的学生每学期不超过二人次,班级财产保护好。

5、学生巩固率为100%,班级无学生流失。

6、工作作风民主,学生及家长反映好,调查问卷满意率达60%以上。积极与学生家长沟通,每学期家访次数达到十次以上。

7、按学校要求,积极组织学生参加学校各项活动,升旗仪式、校会、班会、两操一课及其他外出参观活动,跟班管理学生,无迟到、缺席情况。寒暑假协助学校组织学生参加社区或社会实践活动。

一、评选目的:

为了进一步加强教研组在学校教育教学和教研工作中的主导作用,充分发挥教研组的作用,营造良好的教学和教研活动氛围,扎实地开展教育教学和教研工作,特制订此方案。

六、评比条件:

(一)教研组建设

1、团队精神。

有共同的奋斗目标,(团队共同制定短期、中期、长远目标),并能为此奋斗目标而努力。

注重团队的力量,不提倡个人英雄主义,组内成员相互团结,真诚互助,不推诿、不发牢骚,主动承担责任,组内气氛和谐、融洽。成员富有团队精神、合作意识,大局观念突出,不说不利于团结的话,不做不利于团结的事。认真参加学校组织的政治业务学习,顾全大局,服从学校工作安排,服从教务处调课、带课安排。整体团队服从学校临时工作安排,积极参加学校组织的各项活动。

2、工作环境。

(1)教研组管理制度、创建优秀团队的方案、奋斗目标上墙;

(2)严格按规范操作使用电脑、饮水机等易燃电器设备。在离室、离校前检查并关闭所有电源,检查是否关闭。不在办公室上网做无关教研的活动(如聊天、玩游戏、炒股、看影视剧、购物等),不在办公室睡觉、抽烟;

(3)按时上下班,无故不迟到,不早退,不私自调课、占课,自觉遵守请假制度,上班时间不串岗闲聊,不高声喧哗,不在办公室处理与工作无关的事。

3、工作态度。

保持良好的状态和工作效率,即使在最艰苦、任务最繁重的时期,也要保证良好的精神状态,不乱发牢骚,传谣言,搬弄是非。关心学生成长,尊重学生,关心学生的身心健康和心理健康,与学生谈话时讲究方法和文明用语。尊重家长,接待学生家长来访温文有礼,交谈时语言得体,对家庭教育不当的家长要正面引导,不批评责难,挖苦讽刺,不在家长面前斥责学生。

4、集体荣誉。

组内成员要有强集体荣誉感和较高的凝聚力,以学校的发展为重,对团队的荣誉负责,团队开展活动时,不要计较个人得失,不要在乎谁是主角,谁是配角,要有敢当小草、甘为人梯的精神。优秀团队的组内成员必须要维护学校整体利益,服从学校工作安排,并能正确处理个人利益与集体利益的关系。

(二)团队业务能力的提高

1、讲学习:组内成员必须完成学校的各项培训任务,定期组织组内开展针对性业务学习,形成良好的学习风气,积极参加集体教研和听评课,踊跃投身读书、交流活动;认真备课,按时上、下课,加强自我反思,积极撰写读书笔记、札记随笔。注重学习过程的管理,通过高质量的随堂听课、读书笔记、学习心得、集体讨论、教后感(反思)等方式提高学习质量,不断增强自身素质,获得个人与同伴的共同提高。与其他组相比效果更优。

2、讲钻研:按时开展教研活动,教研氛围浓,有针对性、有记录、效果好,课题研究有计划、有内容、有资料、有成果。教研活动时,本组教师积极主动、人人参与,组内成员互帮互助,集体备课时,组内成员积极献言献策,教学成果(反思、教案、教学案例等)共享,开展研讨交流活动的氛围较好。

(三)工作业绩:组内成员勤奋工作,敬业爱岗,教育教学业绩突出,在各项各类竞赛中成果显著。严肃、认真、保质保量地完成各项常规工作。努力构建“有效课堂”,大面积提高教育质量,所教每个学生都获得进步和提高。学科组在教育教学实践中凸显课改理念,充分利用现代教育技术为教育教学服务。组内教师参加课堂教学大赛获奖,教学设计为本组集体智慧的结晶,为学校争得荣誉的优先考虑。

(四)校内外刊:完成学校规定的任务。

七、创建和评选办法

1、各教研组都必须参加创建活动,每学期为一个周期。每学期开学,各教研组向学校提交书面的“优秀团队”创建方案。方案包括创建目标、奋斗方向、内容、措施等,并附上所有成员的亲笔郑重签名。方案要求用a4纸打印,在空白处或另附纸签名。上报纸质方案的同时,要将方案以word文档格式发送至指定电子邮箱。各组的创建方案要通过板报橱窗、校园网站等渠道进行公示,欢迎全校师生、家长以及社会各界人士予以监督。依照上述四个方面的创建内容,扎扎实实地开展创建活动。

2、每学期进行一次评比。按照四个方面的要求,采用平时自评的方式,认为本组够条件每学年班组组长申报“优秀团队”,做创建工作报告,教职工大会评议,票数超过95%以上,就认定该组为 “优秀团队”达标班组。一学年两学期达标后方可获“优秀团队”荣誉称号和奖金。

3、实行分管领导捆绑负责制。分管领导要全面参与教研组的“优秀团队”创建活动,加强督促、指导和帮助,与教研组一同进行评价、奖惩。

八、出现以下情况, “优秀团队”评比资格一票否决。

1、组内教师中有乱收费,或其他重大失误,违反校纪校规给学校声誉造成恶劣影响的;

2、组内教师有体罚或变相体罚学生等违反师德问题,并造成较坏影响,受到严肃查处的。

3、组内有拒不接受学校布置任务的现象。

4、组内在出卷、监考、阅卷工作中有舞弊行为的。

5、组内有上课不认真,课堂教学秩序混乱、坐着上课、私自调课、旷课、上课接电话的。

九、奖励:

1、获得“优秀团队”奖的组内成员每人颁发含全组成员合影的奖状,学校拍摄集体照在橱窗中展出;

2、被评选为“优秀团队”的教研组,学校给组内每一位教师奖励现金100元;

3、被评选为“优秀团队”的教研组,由学校推荐参加师或团优秀班组的评比活动。

十、宣传工作:

1、以教研组为单位开展学习,深刻学习和领会创建“优秀团队”的重要性和意义。

5.积极参加社会志愿者服务活动,按时完成课时量。

7.体育成绩达到优良(免修除外)。

1、个人素养30%主要是对候选先进个人者在时政知识、生活常识、艺术素养等方面的书面考察,满分50分,折合30%算入总成绩。

2、教师评议20%请候选人的相关所有任课老师对候选人的课堂、日常表现进行打分,从学习成绩、思想品德、行为规范与班级工作四个方面分别打分。

3、体育测试30%候选者必须通过以下体育测试:50米跑、立定跳远、投实心球、800米耐力跑。应试者在前三项中选择两项,800米耐力跑为必考。

4、个人材料展示20%每位先进个人候选者需为自己制作一份个人材料作为全校展示,并接受评审组老师的打分。

为了提高员工的积极性、提高员工工作的效率,除了对员工在工作中表现突出的给予奖励外,每月评选一到两名月度优秀员工,每年评选一名年度优秀员工,加强员工的积极性和竞争意识。

1、当月出勤率为全勤。(当月无迟到、早退、旷工;请假时间总和不超过8小时。)

2、当月考核得票次数最多。(考评明细见附表)

3、对本职工作积极主动,认真热心,成绩显著受到部门领导的一致好评。

4、对公司忠诚,对同事团结,对领导尊重,对客人礼貌。

5、工作区域干净整洁。

6、当月无惩处记录。

1、至少两次以上月度最佳员工。

2、保持积极的工作态度,对公司有忠诚度及奉献精神。

3、全年无警告以上惩处。

4、对本职工作积极主动,认真热心,成绩显著得到各部门所有干部的一致认可。

1、每月被评为月度优秀员工嘉奖100元。

2、年度优秀员工将进行现金奖励500元。

每月30号各部门所有员工投票选举,选举出票数最高的前5名员工,部门主管(生产经理)对这5名员工进行最终评选,并交总经理审核。次月5号将最终评选的结果张贴通报表扬。

年度优秀员工:由各部门负责人于每年年底12月20号将年度优秀员工名单提报至总经理处审批,经理对该员进行综合观察,如符合公司年度优秀员工标准的,由总经理对该员进行实物奖励或现金奖励。并于公告栏上公布。

为充分调动我区广大社会工作者的工作积极性,吸引更多优秀人才加入到社工队伍当中,促进社工队伍的发展壮大,推动xx区社会工作的发展,经南海区民政局批准,南海区社会工作协会将举办“xx区xx年优秀社工评选活动”。具体工作方案如下:

(一)优秀实务社工(8名):在南海区级或各镇(街道)登记备案的从事社工实务服务的社工机构、社会服务单位(指承接政府资助专业社工服务项目的单位,下同)、社区社会组织、事业单位等从业人员,其中机构/单位管理人员1 名、项目负责人3 名、一线社工4 名;

(二)优秀行政社工(7名):南海区级或各镇(街道)职能部门、村(居)委会、事业单位等从事社会工作领域行政或研究工作的专职工作人员。(社会工作领域指在社会福利、社会救助、慈善事业、社区建设、婚姻家庭、精神卫生、残障康复、教育辅导、就业援助、职工帮扶、犯罪预防、禁毒戒毒、矫治帮教、人口计生、纠纷调解、应急处置等领域从事专门性的社会服务工作。)

(一)基本条件

1.热爱祖国,遵纪守法,热爱人民,热爱社工;

2.认同专业社会工作价值理念;

3.在本地区、本行业有良好影响,得到广泛赞誉和认可。

(二)具体条件

1.优秀实务社工

(1)取得全国社会工作者职业水平证书;

(2)在社工机构、社会服务单位、社区社会组织、事业单位等连续从事专业社会工作实务2 年及以上(其中机构/单位管理人员为3 年及以上),成绩显著;

(3)社会工作专业理念和知识受到同行认可,专业服务水平获得服务受众好评(其中项目负责人:专业社会工作能力和项目管理能力得到本地区同行以及机构、同工认可,专业服务质素令服务受众满意;机构/单位管理人员:专业社会工作能力、机构管理能力、社工人才培养能力得到本地区本行业广泛赞誉,获得机构及同工的认可)。

2.优秀行政社工

(1)取得全国社会工作者职业水平证书;

(2)在xx区级或各镇(街道)职能部门、村(居)委会、事业单位等连续从事社会工作领域行政或研究工作2 年及以上;

(3)社会工作专业理念和知识受到同行认可,服务水平获得服务受众好评,在推动社会工作行业发展上成绩显著,对本地区本行业有良好影响。

1.各镇(街道)组织及发动辖区的职能部门、村(居)委会、事业单位、社工机构、社会服务单位、社区社会组织等积极推荐优秀社工从业人员参与本次评选活动。

2.各镇(街道)社工主管部门统一收集本辖区内优秀社工候选人推荐表,对候选人资料进行甄别和核实,确保候选人资料的真实性,并符合参选要求。同时按评选名额和评选条件,分别在不同领域、岗位对候选人进行初步评选。经初步评选产生的候选人的推荐表及其他推荐材料(电子版及纸质版)于xx 年2 月13 日前上报区社会工作协会。每个镇(街道)上报的优秀社工候选人名额不超过5 名。

3.区级各有关单位和事业单位积极推荐优秀社工从业人员参与本次评选活动,并将候选人推荐表及其他推荐材料(电子版及纸质版)于2 月13 日前上报区社会工作协会,每个单位上报的优秀社工候选人名额不超过1 名。

4.主办单位组织专家评审小组,对优秀社工候选人进行集中讨论评选,通过无记名方式进行投票;同时增加公众评审,通过区民政局网站对优秀社工候选人进行公众投票;其中专家评审小组评分占xx%,公众网上投票评分占xx%;两者得分相加按排名确定15 名“xx区优秀社工”和若干名“南海区社工之星”(xx区社工之星是指具备成长为xx区优秀社工潜质的人员)。

5.在区民政局、区社工协会网站、微博等对评选结果进行公示。

6.表彰嘉奖“xx区优秀社工”和“xx区社工之星”获奖者。

注:其他推荐材料包括:参选者个人事迹(列项表述,800字或以上,要求真实、典型,切实反映专业社会工作能力)、相关证明资料(如社工证复印件及其他证明材料)。

教龄在3(含3年)年以下的本校教师。

1、普通话水平达标并做到一贯使用普通话授课;粉笔字、钢笔字书写规范、美观;能熟练运用现代教育技术,熟练地制作并利用多媒体课件上课(25分)。

2、课堂教学中能体现“三段式”教学策略当前研究的重点,成效显著或形成可推广的经验做法(25分)。

3、模范执行教学常规。认真撰写教案,认真批改、讲评作业,在教务处和教学部常规检查中获得好评(25分)。

4、教学质量不断提高,在学校组织的学科统考中,教学成绩b(含b)级以上(25分)。

1、开学第三周起,以年级为单位,举行教学基本功比赛,符合条件的教师均应参加,比赛成绩作为评选教学新秀的依据。

2、开学第三周起,开展公开课评比活动,采用教研组-年级-学校逐级评比的办法,各教研组、年级、教务处应分别在一周内、六周内、期中考试后四周内完成公开课评比活动。

3、各年级、教务处每学期期中、期末考试前一周进行教学常规检查,结果作为评选教学新秀的依据。

4、每学期末,教务处根据参评教师在上述各项活动中的成绩和期中、期末教学实绩,评出学校十佳教学新秀。(当符合条件的参评教师超过30人时,按30%比例选出)

1、学校对评出的十佳教学新秀授予“十佳教学新秀”荣誉称号,颁发证书。

2、年终考核,该荣誉按地级荣誉计分。

3、市级以上教学新秀评选,候选人按名次从中选出。
